St. Joseph's Community Home Care is designed to help patients remain healthy, active, and independent in the comfort of their own home.  Our services are delivered with St. Joseph's long-standing commitment to quality at competitive rates.  All medical care plans are physician approved and superviced by a registered nurse.  Nursing assessments are provided and individualized care plans are designed at the start of care to maximize patient benefit and satisfaction.

Who Can Benefit from Home Care?

• Those discharged from a hospital or nursing home that still need additional care at home
• People who require extra help due to chronic health problems or medical conditions
• Individuals who need long-term intravenous therapy for nutrition or medications
• Individuals on ventilators or other medical life support who perfer a home environment
• People who need short-term assistance at home because of same day or outpatient surgery, or other temporary needs

Home Care Professional Staff

Professional home care services are provided by registered nurses (RNs), licensed vocational nurses (LVNs), therapists, medical social workers, and home health aides.

Personal Care and Respite Services
Home Health Aides provide assistance with bathing, dressing, meal preparation, light housekeeping, general care needs, companionship and more.

Skilled Care
Registered Nurses and Licensed Vocational Nurses provide assistance with intravenious (IV), oral and injected medications, wound care, dressing changes, catheter care, feeding tube and tracheotomy management, ventilator care an more. Therapy and social work services are also available.

Flexible Scheduling

Home Care Services are available seven days a week and allow flexible schedule to meet the needs of our patients.  Services can be provided on an hourly basis (minimum of 4 hours), or by the visit.
